About the job

We are looking for qualified (HCPC registered) Social Workers or OT’s who are not afraid to do things differently to create support that is better for people and lead services through times of improvement and be able to demonstrate the team are meeting quality and performance targets.

You will work a flexible shift pattern across 7 days a week, Mon-Fri 8.30am-8pm and Sat and Sun 9am-4pm and you will be responsible for providing management cover and guidance to the team outside the normal office hours.

You will work closely with community and the third sector, as well as health colleagues to provide better outcomes for people and improve the patient experience, working as part of our multi-disciplinary team, supporting people and families to agree timely discharge plans and advising on creative solutions to help people return and remain at home. You will lead the team and liaise with key partners to facilitate improved provision of services, team training and recruitment.

You will use data and customer experience to demonstrate that the transformation is helping and improving outcomes for all.

You will influence the development of our adult social care service, you will bring your ideas to the table and you will be encouraged to make improvements to ways of working.

About you

You’ll be brave in your approach to work, not afraid to do things differently where it genuinely benefits the service user and you’ll be able to provide inspirational leadership and demonstrate a real creative spark when it comes to your work.

Primarily, it will be up to you to oversee the delivery of our service in a specific location within Somerset and we’ll look to you to lead a multi-disciplinary team, motivating and managing all members to ensure that their work is of a consistently high standard. Promoting independence should be the driving factor in everything your team does, and we’ll look to you to ensure that this is always the case.
