About the job
We are looking for qualified (HCPC registered) Social Workers who are not afraid to do things differently to create support that is better for people.
You will work a flexible shift pattern across 7 days a week, Mon-Fri 8.30am-8pm and Sat and Sun 9am-4pm with your future colleagues who are friendly and very supportive. The 7 day service began operations in October 2018 so you have the opportunity to work hours that suit you as long as the new out of hours service is covered.
Working as part of the multi-disciplinary team, supporting people to get back to their home whenever possible and as quickly as possible as soon as they are able.
You will be expected to work with patients, families and our community networks to agree creative solutions to help people return and remain at home. You will work closely with health colleagues to provide better outcomes for people and improve the patient experience.
You will provide really useful information and advice to people and their families and you will have conversations with people to identify what they are worried about or what they want to achieve, what support they already have and what they need. You will advise them on the best local services and solutions to suit the person’s needs. It is your role to encourage people to be independent, to help them grow their confidence and make the most of the support available in their local community.
You will make connections in the area that you are based, and you will work in partnership with local organisations to achieve the best outcomes for local people.
You will be expected to be flexible in your work base and to be willing to travel within the area of Somerset where you work, you will be covering the acute hospital as well as the community hospitals in the area and potentially working within admission avoidance as well.
